The very best care is centered around you and your unique health needs.

Unfortunately, when multiple healthcare providers (from primary care to specialty care) work independently, with little communication or shared information, the result may be fragmented care which may not deliver optimal quality and value.

Thankfully, with a Medicare ACO like Qualuable, over time, the care you receive should be more responsive, centered on you. You see, healthcare providers participating in an ACO communicate with you and with each other to make sure that you get the care you need when you’re sick and the support you need to stay healthy and well.

In fact, each Qualuable ACO provider participant believes in shared responsibility, accountability and the importance of forming a partnership with you.

Together, they will encourage you to take an active role in your health care, giving you access to tools and services created to provide improved care, especially for those with complex medical needs or chronic conditions.

With immediate access to certain electronic health information about you, such as your medical conditions, prescriptions and medical visits, your Qualuable provider will be fully aware of the totality of care you’re receiving from others within the Qualuable ACO. Each of your health providers will not only know about the medical issues that they have treated, they will have a more complete picture of your health through communicating with your other health care providers.

In short, with doctors who are working together, sharing information about you, you’ll enjoy high-quality, patient-centered care that meets your unique individual needs and preferences.

It is important
for you to
several things:
  • Qualuable isn't a Medicare Advantage plan or an HMO plan.
  • Your Medicare benefits are not changing.
  • You still have the right to use any doctor or hospital accepting Medicare.
  • All of your personal health information is kept private and protected by federal law. However, if you do not want Medicare to share your information with Qualuable, you must call 1-800-MEDICARE (1-800-633-4227). TTY users please call 1-877-486-2048.
